B34610 – Mary Hopkin Signed 1969 AFTRA Pension & Welfare Contribution Form (USA)

An original AFTRA authorisation form for the payment of pension and welfare which has been signed by Mary Hopkin in black pen. The rest of the form has not been completed and it has been stamped as ‘Received’ with the date 17^th October 1969 below. It measures 21.6cm x 28cm (8.5 inches x 11 inches). Three holes have been punched along the top edge and two have been punched along the top edge. There are pin holes to the top left corner and there is some light creasing to the edges. The condition is very good plus.
